Achilles Tendonitis The Facts

Overview

Achilles TendonAchilles tendonitis is an inflammation of the Achilles tendon. This inflammation is typically short-lived. Over time, if not resolved, the condition may progress to a degeneration of the tendon (Achilles tendonosis), in which the tendon loses its organized structure and is likely to develop microscopic tears. Sometimes the degeneration involves the site where the Achilles tendon attaches to the heel bone. In rare cases, chronic degeneration with or without pain may result in rupture of the tendon.

Causes

Achilles tendonitis is aggravated by activities that repeatedly stress the tendon, causing inflammation. In some cases even prolonged periods of standing can cause symptoms. In many people who have developed achilles tendonitis, chronic shortening of the gastroc-soleus muscle complex is the reason that home remedies and anti-inflammatory medications fail. In these instances the muscle itself becomes shortened and creates a constant stress at the tendon?s attachment. Like a green branch that is slowly bent, eventually it begins to breakdown. Over a prolonged period the tendon becomes inflamed, and in the worst cases, appears swollen and thickened. In certain circumstances attempts to heal have failed and the body?s inability to heal the tissue results in degenerative changes known as achilles tendonosis. Anti-inflammatory medication, stretching and ice may only provide temporary relief, because they address the inflammation but not the root cause.

Symptoms

Mild ache in the back of the lower leg, especially after running. More acute pain may occur after prolonged activity, Tenderness or stiffness in the morning. In most cases the pain associated with Achilles tendinitis is more annoying than debilitating, making sufferers regret activity after the fact, but not keeping them from doing it. More severe pain around the Achilles tendon may be a symptom of a much more serious ruptured tendon.

Diagnosis

The diagnosis is made via discussion with your doctor and physical examination. Typically, imaging studies are not needed to make the diagnosis. However, in some cases, an ultrasound is useful in looking for evidence of degenerative changes in the tendon and to rule out tendon rupture. An MRI can be used for similar purposes, as well. Your physician will determine whether or not further studies are necessary.

Nonsurgical Treatment

Physical therapy is the first and most useful defense for achilles tendonitis because of the two presentations outlined above. Treatments for the two types are quite different in approach. Midsubstance tendinitis responds well to stretching, whereas insertional tendnitis tends to be aggravated more by it. Depend on your trusted physical therapist to differentiate between the two and follow their guidelines on exercises and running modifications. Running gait patterns that show excessive ?sinking postures? tend to point to the source of achilles tendon problems. Altering your gait in the midstance phase of the cycle can reduce the load on the tendon dramatically and thereby reduce pain. Rely on your running physical therapist for proper guidance on altering your gait the right way. Stride Strong?s Portland Running Clinic gait analysis can identify and fix potential issues before pain sets in. Icing at the onset of acute achilles pain (i.e. when the injury is fresh and new) would help control the inflammation. Your next step should be to call our number for an appointment.

Achilles Tendon

Surgical Treatment

Surgery is an option of last resort. However, if friction between the tendon and its covering sheath makes the sheath thick and fibrous, surgery to remove the fibrous tissue and repair any tears may be the best treatment option.

Prevention

There are several things you can do to reduce the risk of Achilles tendinitis, warm up every time before you exercise or play a sport. Switch up your exercises. Slowly increase the length and intensity of your workouts. Keep your muscles active and stay in shape all year-round. When you see symptoms of Achilles tendinitis, stop whatever activity you are doing and rest.

During the golf swing the body acts as a whip, power production starts with the feet pushing against the ground. Maintenance of proper foot alignment on the back swing is critical for control of the downswing and contact position. The front heel occasionally comes off the ground to promote a full shoulder turn. Golf should always be played from the insides of the feet.

As you can imagine, healthy feet are critical to a successful golf game.

Sprain: When the fibers of the tissue tear due to overstretched ligaments, a sprain occurs, and a sprain is known to cause extreme pain. Muscle Injury: Overloading, overuse, over-stretching, or bruising of the muscles and tendons of the foot may lead to severe pain in the foot. Plantar Fasciitis: Planter fascia strains are generally felt as the pain at the bottom of the foot. This condition is known as plantar fasciitis. Wrong Shoes: Wearing narrow toed shoes may lead to development of a bunion, a big, hard, and painful protrusion at the base of the big toe, causing pain in the foot. Hammer Toes: As contraction of muscles leads to hammer or curled toes, it is often a cause of severe pain.

Athletes foot is a common skin condition causing itchy, flaky skin and a burning sensation usually between the toes. Flat feet are usually the result of one's own genetics inherited from their family. Flattening is a normal part of the walking cycle of the foot, and in fact this is how the body disperses much of the shock forces created with walking. However, in some individuals, the foot flattens outward too much. This changes the way certain muscles in the foot and leg have to function, which causes numerous changes to the feet over time. These changes can include chronic straining of ligaments and tendons, as well as the development of deformities that rely on structural imbalance like bunions and hammertoes. There are numerous ways to treat flat feet.

They can be treated using foot supports, heel pads, and heel cups. In order to retain healthy feet, you must make an effort to take care of your feet. Wash your feet every day in lukewarm soapy water but do not soak the feet as this can cause excessive dryness of feet. Dry your feet completely, especially in between the toes and moisturizing cream should be applied all over the feet, but not in between the toes. You may smoothly rub with a pumice stone or a skin file. Trim them straight across the toe and not too short. Regularly scrub your feet, mainly the heels, using a foot scrubber. It helps in removing the dead cells and making feet softer. Examine your feet regularly with mirror and if there are any defects or infections, then immediately consult a doctor. Never do self-treatment for feet. A toe corn could develop on the top of a toe or between toes.

Calluses typically develop under a metatarsal head (the long bone that forms the ball of the foot) that is carrying more than its fair share of the body weight, usually due to it being dropped down or due to its longer length. Begin by soaking your feet in warm soapy water and gently rubbing away any dead skin that loosens. A pumice stone or emery board is then used to "file" this thickened skin. Apply a good moisturizer to the hardened areas to keep them softer and relieve pain. Non-medicated corn pads or moleskin (a thin fuzzy sheet of fabric with an adhesive back) can relieve calluses, but should be removed carefully to avoid tearing the skin. Calluses can be trimmed and comfortable padding applied to these painful areas. In addition to medication to relieve inflammation, cortisone may be injected into the underlying bursal sac to rapidly reduce pain and swelling. As a result, the skin under this bone thickens like a rock in your shoe. Improperly fitting shoes are a leading cause of corns. The result is a foot ulcer.

Orthotics, on the other hand, do most of their work on the heel and ball of the foot repositioning the foot for maximized function. First, try doing your own version of deep tissue massage by rolling a frozen cola bottle or can from the heel forward into the arch. Do stretching but the key to good stretching is not to stretch too hard so generally avoid weight bearing (standing) stretches but sit on a soft surface like your bed and pull the foot backward on the leg as far as it will go, holding for 20 seconds and relaxing for 5 seconds. Wrap a pillow around your foot with duct tape and walk for a block or two. You will come back with your foot hurting more because your foot sank down deeper into the soft surface, allowing the ligament to stretch more. For myself, when I'm in a hurry, I use a foot file from Sally's.

Caring For Your Feet As You Age

Anyone who's ever played hide and seek knows what a thrill it is to be well-hidden somewhere, perhaps only a breath or two away from the soft shoe scuffs of the seeker, almost bursting out laughing because you know if they just twitched the curtain aside, took one step behind them, or shifted the branch slightly they'd see you, grinning, right before you dashed off to whatever upended pot, tree stump or floor-strewn sweatshirt was 'safe.' Possibly jealous of these childhood exploits, the arches in your feet may want to get in on the hide-and-seek action.

Podiatrists treat bunions, hammertoes, and all sources of toe and forefoot pain more than any other condition, save for heal pain and nail disease. There can be many complex mechanical causes for these conditions, as genetics has only an initial role in most cases of bunions and other toe and foot deformities. These conditions are successfully treated all day long by podiatrists, and should be the obvious first choice in care when foot pain develops. Sprains are a common injury, and often occur in the evenings or weekends after most medical practices are closed. It is very appropriate to present to an urgent care center or emergency room for serious foot and ankle sprains to ensure there is no fracture. These products can burn your skin.

Skin conditions that involve open sores, lesions, or contagions may also be treated with medical ointments and bandages or wraps. Skin disorders that are temporary and merely cosmetic in nature can often be treated with medicated make-up, over-the-counter skin care products, hygiene techniques, and small lifestyle changes. In addition, some skin conditions can be treated or improved with changes in diet. Some skin disorders cannot be prevented; genetic conditions and disorders that are brought on by other illnesses cannot be avoided. However, it is possible to prevent some skin disorders. Learning about proper skin care and skin disorder treatment can be very important for skin health. Some conditions require the attention of a doctor, while others can be safely addressed at home.

Go for those, which provide support, cushioning, and enough room for the toes to move. People with flexible flat feet have arches that disappear when they put weight on their feet, but which reappear when the feet are not weight-bearing, or when they go up on their toes. In fact, this reappearance of the arch while the foot is non-weight bearing is really what separates this type of flatfoot from other types. It's as though the arches take toe-standing as a general call of olly-olly-oxen-free: time to come out and tease the seeker about how great your hiding place was. When a patient suffers a foot or lower leg injury they should see a podiatrist as soon as possible to receive the appropriate advice and treatment. The podiatrist will need to understand the cause of the injury, any previous injuries and the level of activity prior to the injury occurring. A comprehensive biomechanical assessment of the patient walking or running will then be carried out to outline any issues with foot/knee or hip alignment that may be causing or contributing to the condition. Podiatrists care for any skin and nail problem involving the feet. The skin may turn red, and start peeling.

Footwear can be modified with stretching over the protruded area to further reduce pressure and in extreme cases footwear can be modified with a balloon patch. In this case, the Pedorthist would cut a hole in the upper of the shoe at the bunion area and place a patch over the hole to create more space for the bunion. Orthotics in this case can help to bring the foot into a more neutral position therefore reducing pressure on the joint and resulting pain. It should be noted that orthotics should always be put into proper fitting footwear to achieve the best results. Proper footwear is the first step in the treatment of a neuroma.
